On the evening of January 2, Japanese media reported that Japan Airlines Flight JAL516 caught fire after landing at Tokyo Haneda Airport.
It is known that this plane was carrying 379 passengers (including crew) at the time of the incident.
After receiving the tip, firefighters were on the scene to extinguish the fire but it was extremely difficult. Fortunately, all passengers on board were safely evacuated using an emergency slide so there was no major loss of life.
After extinguishing the fire, the Japan Airlines plane was left bare, desolate.
According to the initial investigation, the cause of the incident was that a Japan Airlines commercial plane collided with a Japan Coast Guard Coast Guard aircraft. The coast guard plane was preparing to carry relief supplies to the earthquake zone on Japan's west coast. The crash killed 5 out of 6 people The Coast Guard plane was killed.
In a statement announcing the incident, Japan Airlines said it had obtained permission from air traffic control before a collision with a coast guard plane at Tokyo Haneda Airport on the afternoon of Jan. 2.
Cockpit recordings have also recently been released showing that the crew of JAL516 was given permission to land on runway 34. The crew reiterated the command to land on runway 34 in the recording.
In an air traffic control tower recording allegedly recorded moments before the horrific collision played on a website broadcasting live air traffic signals, a voice can be heard instructing the Japan Airlines plane to continue landing.
On January 3, representatives of the French Bureau of Investigation and Analysis of Civil Aviation Safety sent 4 investigators to Haneda Airport to participate in an investigation organized by Japan's Transportation Safety Board.
Japanese broadcaster NHK quoted officials from Japan's Ministry of Land, Infrastructure, Transport and Tourism as saying the cause of the accident at Haneda Airport could have been "the Coast Guard (JCG) captain misunderstanding instructions from air traffic controllers."
Accordingly, air traffic control at Haneda Airport allowed the Japan Airlines plane to land and asked JCG's Bombardier DHC-8-315Q patrol aircraft to stop on the taxiway close to the runway to avoid the risk of collision. However, Coast Guard captain Genki Miyamoto, 39, apparently misunderstood the air traffic controller's instructions and informed JCG that he had been "granted permission to enter the runway for takeoff." Since then, there has been a horrific collision between 2 aircraft.
Paul Hayes, director of aviation safety at UK-based consultancy Cirium Ascend, commented: "The obvious question is whether the JCG aircraft entered the flight path, if so, why?"
Aviation expert Mike McCarron said the crash was likely caused by human error, not a technical glitch. He said: "Obviously the problem here is that someone showed up in their wrong place. The (Japan Airlines) flight is about to land. Apparently this plane was allowed to land. So what are Coast Guard planes doing there and why they're on the runway, that's the big question right now. Did air traffic control make a mistake? Do airplane pilots make mistakes? We have to find out where the error is and take steps to ensure that these things don't happen again."
In addition, experts also pointed to a number of possible causes of the accident such as poorer night vision, the small size of the Dash-8 patrol aircraft, obscured lights inside the Dash-8 aircraft, communication errors, errors in recognizing signs on the runway.
Former British Airways aviation safety chief John Lindsay said "a string of mistakes", not just one, led to the crash. However, John Lindsay acknowledged that there is still no evidence of what happened.
