Adapted from the hit Korean work of the same name, Heart Without Fake also brings together the cult stars of the Golden Pagoda such as Baifern Pimchanok, Win Metawin, Sapol Assawamunkong and Jane Ramida Jiranorraphat. The story revolves around Liu (Baifern Pimchanok) – a girl who was mocked in high school for her poor appearance, only to decide to have plastic surgery on her entire face to achieve her ideal beauty.

Liu's new life seems to take a turn after meeting her first friend Faye (Jane Ramida), a confident girl with natural beauty. However, things get complicated when Faye gets to know Guy (Win Metawin), a handsome celebrity boy who is also a former classmate of Liu's in middle school. With a straightforward and kind personality, Guy realizes that Liu is an old friend and warns her about Faye, which makes Liu hate him even more.

Meanwhile, Liu finds solace in Saint (Sapol Assawamunkong), a graduate student who is always by her side and supports her. Liu's new life gradually becomes peaceful until an unexpected situation forces her to reveal her "cutlery" face. The truth makes everyone around him talk and criticize.

It is worth noting that the age gap between the main couple in the Thai version is similar to the Korean version - both 7 years apart. In the original version, actress Im so Hyang was born in 1990 and actor Cha Eun Woo was born in 1997. Similarly, in the Thai version, the female lead Baifern Pimchanok was born in 1992, while the male lead Win Metawin was born in 1999.
