According to Variety, the plane crashed at noon on Thursday while carrying father and son Christian Oliver from Bequia Island in the Grenadines Islands to St. Lucia Island. "Shortly after takeoff, the aircraft malfunctioned and plunged into the sea," St. Vincent and the Grenadines Royal Police said in a statement. Police said fishermen and divers assisted in the search as soon as they received word of the accident. Christian Oliver, his children and the pilot were found at the scene by the Coast Guard. He saw the plane crash, then it regained its balance and flew back. However, only a moment later, it plunged into the sea. "When the plane sank, it made a 'Bouff!' sound. That's why I thought it exploded," Cornell Campbell said.

Another witness, Tyrone Durham, a retired coast guard officer, said the plane may have suffered engine failure. "It looks like it's coming back to the airport," he said. "But it was controlled by the wind and fell into the sea."
